Registration Information

New Student Registration

Students entering a Florida school for the FIRST TIME, at any grade level, will be asked to present the following items upon registration:

· current immunization dated and signed by a licensed health professional
· proof of a physical examination dated and signed by a licensed health professional within the last 12 months or less than one year from the first day of school
· birth certificate
· Social Security Card (optional)
· proof of Florida residency (utility bill, lease agreement)
· copy of student’s last report card (if applicable)
· legal guardianship records (if applicable)

Please note: Students entering kindergarten for the first time must be 5 years of age on or before September 1st.

Students transferring from another Florida county, at any grade level, will be asked to present the following items upon registration at the school:

· current immunization records
· Social Security Card (optional)
· proof of Florida residency (utility bill, lease agreement)
· copy of student’s last report card (if applicable)
· legal guardianship records (if applicable)

Attention to Anyone Born in Puerto Rico: Puerto Rico’s New Birth Certificate Law

As of July 1, 2010 it will be necessary for all citizens born in Puerto Rico to be issued a new official birth certificate. Citizens living in the states will be able to request and receive their new birth certificates through a simple mail process.
